• Scale – Flake arising from horny layer, indicate inflammatory changes and thickening of epidermis• Keratosis – Localized horny overgrowth of the skin, such as wart or callus.• Crust – May look like a scale but is composed of dried blood or tissue fluid• Ulcer – Whole epidermis and at least upper part of dermis is lost, heals with scarring• Erosion – Superficial loss of tissue, much wider than it is deep. Heals without scarring• Excoriation – An ulcer or erosion produced by scratching involving the epidermis• Fissure – A slit in the skin• Sinus – Cavity or channel that permits the escape of pus or fluid• Scar – Result of healing, normal structures permanently replaced by fibrous tissue• Atrophy – Thinning of skin caused by reduction of epidermis, dermis or subcutaneous fat. If epidermis is atrophic it crinkles like cigarette paper• Lichenification – Thick, leathery skin, usually the result of constant scratching and rubbing• Stria – Stretch-marks, streak like linear atrophic pink/purple/white lesion of skin caused by changes to connective tissue• Pigmentation - surrounding skin --> develop after lesion heals
