  • Indikationen der Lungentransplantationen - Unheilbare pulmonal-parenchymale oder vaskuläre Erkrankung im Endstadium  - Lebenserwartung <2 Jahre - FEV1 < 25% Soll - Pulmonale Hypertension - NYHA III oder IV - keine maligne Erkrankung
  • Lungentransplantation Nachbehandlung 1. Medikamente- Immunsuppression (Cortison, Cyclosporin)- Infektprophylaxe  2. Monitoring - Bronchoskopien - FEV1- Exhaliertes NO - Medikamentenspiegel  3. Patientenschulung- Weiterbildungen - Tagebuch- Mikrospirometer
  • ASA-Risikogruppen ASA I: gesunder Patient. ASA II: leichte Allgemeinerkrankung ohne Leistungsminderung. ASA III: schwere Allgemeinerkrankung mit Leistungsminderung. ASA IV: lebensbedrohliche Allgemeinerkrankung.
  • Lungentransplantation Immunsuppresiva 1. Tacrolimus: prä- und postoperativ 2. Mycophenolat Mofetil (MMF): postoperativ 3. Glukokortikoide (Prednison) andere: Cyclosporin, Sirolimus
  • Bronchiolitis Obliterans Syndrom (BOS) = Häufigste Form der CLAD (Chronic lung allograft dysfunction) - klinisches Syndrom definiert über FEV1 (Obstruktion)  - histologische Diagnose - Oft bezeichnet als “chronische Abstossung” - Hauptursache der inferioren Langzeitresultate - Multi-faktoriell
  • Video-assisted thoracoscopic surgery (VATS) Procedure:- Lung wedge excision, pleurectomy - Lobectomy, sympathectomy - Thymectomy, pericardial fenestration  Advantage:- Small incisions, less pain - Small scars (good cosmetic result)  Disadvantage:- Limited exposure- no palpation
  • Anzeige
  • Anterolateral Thoracotomy - zwischen 4. und 5. Rippe (unterhalb des M. pectoralis major) Procedure - Lung resections, tracheal resection - Thoracic esophagectomy Advantage:- Less painful than posterolateral - Cosmetically satisfactory scar  Disadvantage:- Limited exposure of the posterior mediastinum
  • Median Sternotomy Procedure:- open heart surgery- tumors of anterior mediastinum (Thymoma)- tracheal surgery- bilateral pulmonary surgery Advantage:- excellent exposure of heart, anterior mediastinum- reduced pain- fast and easy to teach Disadvantage:- limited exposure of the lung and hilum
  • Robotic-assisted surgery - Thymic pathology - Aberrant parathyroid tumors - Cysts (thymic, bronchogenic, pericardial foregut cyst) - Neurogenic tumors - Germ-cell tumors (hybrid procedure)
  • Primary pneumothorax - without clear cause - no apparent underlying lung disease - „RUPTURE OF A BLEB OR BULLA“ in the apex Typically:- tall thin men - between 20 and 40 - Cigarette smokers (risk 100% higher)  - very high risk of recurrence!!
  • Indication for pneumothorax surgery - Prolonged air leak (> 3 days) - Recurrent episode (>2) - Non re-expansion of the lung - Occupational risk (Taucher, Piloten) --> Thoracoscopic surgery:- Bullectomy + pleural abrasion 95-97% success - Talc pleurodesis: 90% success
  • Tension pneumothorax - Dyspnea- Tachycardia (pulse hard to feel)- Jugular venous distension- Hypotension (life threatening)- Hyperresonance of the chest wall on percussion  --> Chest drainImmediate decompression by insertion of a needle(ca. 4. ICR, ant. axillary line)
  • Pleura - Pressure normally: -5 cm H2O (up to 150 cm H2O) - Pleural fluid: ∼15-30 ml per day and side Composition:- Protein 10-20g/l - Glucose: as in plasma - LDH: < 50% of plasma level - Cells 4500/mm3 - pH = 7.36
  • Empyema - pH <7.20- LDH >1000- Glucose < 40mg/dl or <25% of blood glucose- Pathogenic germs- high white blood count 1. Exsudate phase2. Fibro-purulent phase3. Organizing phase Causes: - pneumonia- thoracic trauma- postoperative complication (zB bronchopleural fistula)
  • Empyema Therapy - One-time thoracocentesis/Chest tube for microbiologic analysis THEN beginning of antibiotic treatment - Fibrinolytic therapy (eg Urokinase) - VATSDecortication (Entschwartung)Thoracoplasty Clagett window
  • Malignant pleural mesothelioma Treatment Multimodality treatment 1. Chemotherapy (cisplatin/gemcitabin) 2. radiotherapy: palliative, local 3. Surgery MCR: macroscopic complete resection - P/D (Pleurectomy/Decortication): Lungenabschnitte gelassen- EPP (Extrapleural Pneumonectomy): Lung, diaphragm, pericard taken

  • Management of BPF (Bronchopleural fistula) - Direct closure (PDS 4.0) - Shortening of the bronchus and closure by stapling or suture - indirect closure with omentum as patch --> Cover BPF with well vascularized tissue (omentum, muscle)
