Gesteigerte Thrombozytose im Knochenmark
- F>M- Zwei Altersgipfel: ca. 30 Jahre und 50-60 Jahre
Klinik:- 30-70% sind asymptomatisch- Häufige Erstmanifestation: Thromboembolische Ereignisse und petechiale Blutungen- Splenomegalie
Diagnose (alle 4 müssen erfüllt sein):1. Sustained platelet count >450 x 109/L2. Bone marrow biopsy: proliferation mainly of the megakaryocytic lineage3. Not meeting WHO criteria for PV, MF, CML, MDS, other neoplasms4. JAK2 or other clonal marker or no evidence of reactive thrombocytosis
Therapie: - Grundtherapie: Aspirin - Zytoreduktiv: Hydroxyurea, Anagrelid (Thrombopoiese-Inhibition)- JAK2-Inhibitoren, allogene Stammzelltherapie
